Founded in 1993, Careteam provided supportive services for people living with HIV.
Today, Careteam provides Chronic Health Care Management to anyone living with a chronic disease.
Careteam staff is proud of what we do and strives always to treat each patient as if they were family.

Founded in 1993, Careteam+ today provides medical and support services for people living with chronic health issues. Through the Ryan White Care Act in 1994, Careteam received its first federal grant to provide medical, treatment and social support services to uninsured clients living with HIV in Horry, Georgetown and Williamsburg counties. For the next 20 years, Careteam has been the primary source of care and services for people living with HIV whom are medically uninsured.

Staff at Careteam has always strived to provide compassionate care, making sure each client is treated with respect and dignity. Through the years with the advances in medicine, Careteam has seen its patients’ health improve.

Many of our patients regained their health, stopped receiving disability and returned to the workforce. HIV is no longer viewed as a terminal disease, but now is viewed as a chronic disease that can be controlled with medication. HIV patients have weakened immune systems and often have chronic health issues, such as diabetes, heart disease and obesity– just like many others.

In response to our patient requests and to ensure continuity of care for those clients that became insured through the workplace, Careteam opened its own medical offices in 2010 to accept insured clients. Appreciating the high level of care and attention to detail they were receiving, clients began asking repeatedly to receive ALL of their medical care with Careteam. In response to those requests, Careteam began providing primary health care to our patients in 2013.

Due to patient and family requests, in 2014 Careteam also began providing primary health care to patient’s families and the community as well. It was then Careteam starting using the name Careteam+ to illustrate the new level of healthcare services available to the community.

Today, Careteam+ offers Chronic Disease Management to anyone living with chronic conditions, including: Arthritis, Asthma, Diabetes, Pre Diabetes, Infectious Diseases (HIV and Hepatitis), Heart Disease (High Blood Pressure, High Cholesterol) Obesity/ Overweight , and help to stop smoking. Our doctors have specialties in Infectious Disease, Preventive Health and Public Health, making them uniquely qualified to treat individuals.

These services are assessed based on need, eligibility and availability of funding.
HIV affects two groups of people more than others—African Americans and Men who have Sex with other Men (MSMs).

1 in 16 African American men and 1 in 32 African American women will be diagnosed with HIV in their lifetime. MSMs ages 13-34 are the only group where infection rates have increased, as much as 25% from 2006-2009.
