As a Community Health Worker with Careteam Plus, I play an active role in bridging the gap between healthcare and the community. I assist with planning and supporting community events that promote health and wellness while providing diabetes education to individuals and groups to help improve awareness and outcomes. I work to develop strategic plans and partnerships that strengthen community outreach efforts and create lasting connections. In addition, I help patients, and their families navigate healthcare systems and access essential community services, ensuring they receive the resources and support they need. My work is centered on building relationships, fostering collaboration, and empowering communities to live healthier lives.
