We at Careteam Plus are thrilled that you have chosen us as your primary care provider! To help make your first visit as smooth and efficient as possible, we ask that you complete the New Patient Forms ahead of time or arrive 15 minutes early to complete them before your appointment.
To assist us in providing you the best possible care, please bring the following to each appointment:
- Let us know where you have received previous medical care.
- Share details of any recent hospitalizations or Emergency Department visits.
- Provide the name and contact information for your pharmacy (if not using ours).
- Bring your medication bottles or a list that includes dosages, frequency, and times.
- Inform us in advance if you require any special accommodations, such as an interpreter.
If you are unable to keep your appointment please give us as much advanced notice as possible to allow us to reschedule you.
Cost
Insurance:
Careteam Plus accepts most major insurance plans. Your insurance card should have your co-pay listed, or you can contact your insurance company to confirm your co-pay.
Uninsured:
Careteam Plus is committed to providing quality healthcare to everyone, regardless of their ability to pay. We offer a Sliding Fee Program for low-income or uninsured patients. The fee is based on income and family size, using the Federal Poverty Guidelines. This fee covers your visit and any standard lab orders.
We also offer Payment Plans for patients facing financial difficulty and Waivers or Fee Reductions in case of income loss due to unforeseen circumstances, such as a natural disaster.
Lab Orders:
Non-standard lab orders will be billed at Careteam Plus’s contracted prices with LabCorp. We encourage patients to discuss the specific charges for labs in advance.
