At Careteam Plus, we believe everyone deserves access to high-quality health care regardless of their ability to pay. We offer a Sliding Fee Scale to help make our services affordable for all.
Our Sliding Fee Scale is based on your household income and family size, following federal poverty guidelines. This means you may qualify for reduced costs for Medical visits and/ Pharmacy services. Even if you are insured, whether you are uninsured, underinsured, or just going through a tough time, we are here for you!
How It Works
- Fill out a Sliding Fee application – Our team will walk you through the process.
- Submit required documents – We will need proof of income and household size.
- We calculate your fee – Based on your income level, you may pay a reduced rate or only a nominal fee for services.
- Reassess Annually – Your application is reviewed annually. However, if your financial situation changes at any time, we will be happy to help you complete an updated application to ensure you continue receiving the support you need.
- Waiver – We also have a waiver for emergency situations where there is a loss of home or loss of income.
