How do I schedule an appointment at Careteam Plus?

What should I bring to my first appointment?

Please bring your insurance card, a list of current medications, and any relevant medical records to your first appointment. You can also fill out any new patient forms ahead of time that can be found here.

Does Careteam Plus accept my insurance?

If you have a question about your insurance, please call the office at 843-234-0005.

How does the sliding fee scale work?

Individuals may apply for the Sliding Fee Scale at any time.  If you experience a significant and long-term change in your employment or household circumstances, you may qualify for a free waiver or reduction. To see if you qualify, please see our new patient forms for more information.

Can I pay my bill online?

Yes! You can pay your bill online if you have an active account on our Patient Portal.

What should I do in case of a medical emergency?

In case of a medical emergency, stop what you are doing and dial 911 or go to the nearest emergency room immediately. While we do offer health care services, we are not an emergency service.
