At Careteam Plus we offer a Free Pre-Diabetes Prevention Program called Life Balance 360. This one-hour interactive class is designed to be fun, informative, engaging, and most of all supportive. Participants gain tools to care for their health every day and reduce their risk of diabetes. You will learn simple ways to manage stress, improve sleep, create healthy habits, and overcome weight-loss plateaus just to name a few topics.

Welcome to Prevent T2! This introductory session helps participants move from thinking about change to taking action. It lays the foundation for the entire Prevent T2 program, a proven course designed to help prevent or delay type 2 diabetes in those at high risk.
By the end of this session, participants will:
– Set simple, achievable health goals
– Understand the goals and structure of Prevent T2
– Learn the basics of type 2 diabetes
– Begin creating an action plan
This session focuses on the importance of physical activity in preventing or delaying type 2 diabetes. Participants will be introduced to simple and practical ways to get active and learn how movement supports overall health.

– Recognize different ways to get active
This session focuses on healthy eating as a way to help prevent or delay type 2 diabetes. Participants will learn the basics of building balanced meals and making healthy food choices.

Identify the items in each food group
This session focuses on the importance of tracking, or self-monitoring, physical activity as a tool to help prevent or delay type 2 diabetes. Participants will learn why tracking matters and how to effectively monitor their activity.

This session focuses on tracking, or self-monitoring, food intake as a key strategy to help prevent or delay type 2 diabetes. Participants will learn why food tracking is important and how to use it effectively, including understanding Nutrition Facts labels.

This session focuses on increasing physical activity as a way to help prevent or delay type 2 diabetes. Participants will learn why becoming more active matters, explore practical ways to increase activity, and understand how to track detailed fitness information.

This session focuses on weight loss as a key strategy to help prevent or delay type 2 diabetes. Participants will learn how weight is affected by calories and how to lose weight by burning more calories than they consume.

Understand how to burn more calories than they take in
This session focuses on healthy shopping and cooking as tools to help prevent or delay type 2 diabetes. Participants will learn how to identify healthy foods and make better choices when shopping and cooking.

Learn how to shop for healthy food
Learn how to cook healthy food
This session focuses on managing stress as an important part of preventing or delaying type 2 diabetes. Participants will learn how stress affects health and explore healthy ways to reduce and cope with stress.

Identify common causes of stress
Understand the link between stress and type 2 diabetes
Learn ways to reduce stress
Explore healthy coping strategies
This session focuses on the challenge of fitting at least 150 minutes of physical activity into a busy week. Participants will learn the benefits of staying active and discover practical ways to make time for fitness.

Identify benefits of being physically active
Recognize common challenges to fitting in fitness
Learn strategies to find time for fitness
This session focuses on coping with triggers that lead to unhealthy behaviors, helping participants prevent or delay type 2 diabetes. Participants will learn how to recognize common triggers and develop strategies to manage them.

Identify triggers for unhealthy food shopping and ways to cope
Identify triggers for unhealthy eating and ways to cope
Identify triggers for sitting still and ways to cope
This session focuses on heart health for people at risk for type 2 diabetes. Participants will learn why heart health matters and how to make choices that protect their heart.

Understand why heart health is important
Learn how to keep their heart healthy
Learn how to make heart-smart choices about fats
This session focuses on taking charge of your thoughts to help prevent or delay type 2 diabetes. Participants will learn how to recognize harmful thoughts and replace them with helpful ones.

Recognize the difference between harmful and helpful thoughts
Learn how to replace harmful thoughts with helpful thoughts
This session focuses on getting support from others to help prevent or delay type 2 diabetes. Participants will learn how to seek encouragement and guidance from people and groups around them.

Learn how to get support from groups, classes, and clubs
Learn how to get support from professionals
This session focuses on eating well away from home to help prevent or delay type 2 diabetes. Participants will learn how to stay on track with their healthy eating goals when dining out or attending social events.

Identify challenges to eating well at restaurants and social events
Learn how to plan for and cope with these challenges
This session focuses on staying motivated to help prevent or delay type 2 diabetes. Designed for the six-month mark, participants will reflect on their progress and plan for continued positive changes.

Identify the next steps for their group
Set personal goals for the next six months
This session focuses on taking action when weight loss stalls to help prevent or delay type 2 diabetes. Participants will learn why weight loss can slow and how to get back on track.
